Breast Pump Benefit
U.S. employees and family members enrolled in the MAP Plus Option 1 Low Deductible and MAP Plus Option 2 High Deductible benefit plans can submit a claim for the purchase or rental of an electric breast pump with the following guidelines:
- Rental or purchase is covered for all eligible members. You are encouraged to be a wise consumer and investigate the cost of rental versus purchase for the amount of time you expect to use the pump.
- Pregnancy related breast pumps are considered durable medical equipment and are covered at 100% under the MAP Plus Option 1 Low Deductible and MAP Plus Option 2 High Deductible plans. No calendar year deductible applies.
- Member must provide:
- Copy of their breast pump receipt
- Aetna Medical Claim Form. Complete only the section titled, "TO BE COMPLETED BY EMPLOYEE/RETIREE."
- Coverage does not cover the cost of repairs or preventive maintenance. Coverage is limited to one pump purchase every 3 years. Please review the Appendix A documents for MAP Plus Option1 and MAP Plus Option 2 for further detail.

Lactation Consultation Benefit
U.S. employees and family members enrolled in the MAP Plus Option 1 and MAP Plus Option 2 benefit plans are eligible for lactation consultation coverage at 100% up to 6 visits every 12 months. No calendar year deductible applies. Consultation must be delivered by a certified lactation consultant.
Member must provide:
- A receipt or invoice from the certified lactation consultant
- Aetna Medical Claim Form. Complete only the section titled, "TO BE COMPLETED BY EMPLOYEE/RETIREE."
